The Department of Transportation (DOT) requires commercial drivers to meet specific health standards in order to operate large motor vehicles.
Companies that employ commercial drivers must ensure their drivers pass a DOT physical exam. DOT physicals give companies peace of mind that their drivers are able to safely operate commercial vehicles while on the job.
The DOT Physical Exam takes under 30 minutes, and a certificate is provided for DOT Patients who meet the required health standards successfully at the time of service. This certificate must then be taken to the MVA or DMV in order to obtain a physical DOT medical card. Once a driver obtains their certificate from Dr. Gonsalves, he/she is deemed fit to drive commercial vehicles, as long as they have a current commercial driver's license (CDL).
We offer DOT Physicals by appointment, however should you walk in, we will make every attempt to fit you in, if the schedule permits.

Each DOT Physical is priced at $100.00 payable by either credit card or cash, for non contracted drivers, and is due at the time of service. Contracted companies may send payment by cash, credit card or company check with their driver, once a signed contract has been received, or may elect to be billed monthly for DOT Physicals for their drivers.

Vision
Hearing
Urinalysis
Blood Pressure
Full Physical Exam
DOT Patients with EITHER of the following conditions MUST be current in their treatment and bring the NAME & TELEPHONE NUMBER of their LOCAL primary care physician so that Dr. Gonsalves may contact them in the event any further information is required to complete the DOT Physical.
DOT Patients with Diabetes must bring their most recent A1C lab results, 1 week's worth of blood sugar readings, and the name and dosage of any medications they are taking to control this condition.
DOT Patients with Hypertension (High Blood Pressure) must bring the name and dosage of any medications they are taking to control this condition.
